The drug Sofradex has a broad spectrum bactericidal and antibacterial effect due to gramicidin and framycetin, and is active against such gram-positive and gram-negative microorganisms.

The anti-inflammatory, antiallergic and antipruritic effect is exerted by the glucocorticoid - dexamethasone. As a result of instillation into the eye, pain, burning, photophobia, tearing and other symptoms characteristic of the pathological processes caused by the corresponding pathogens occur.

Composition and release form

Drops of Sofradex are intended for introduction into the eyes and ears. Therefore, “Sofradex ear drops” is the same as “Sofradex eye drops”.

Composition:

  • Framycetin (5 mg) is an antibiotic that actively influences staphylococcus bacteria, dysentery and other harmful microorganisms (except those that can live in a vacuum-free space).
  • Gramicidin (50 mg) - acts as an additional support for framycetin. The component itself is not expensive, but necessary for the complex treatment of bacterial and infectious diseases.
  • Dexamethasone (500 mg) is a special hormone necessary for the suppression of inflammatory processes. In addition to the main function, it helps relieve allergic reactions, swelling and itching.
  • Additional substances that do not have a medical effect.

Pharmachologic effect

Drug Sofradeks is a combination, and has the following therapeutic effects:

  1. Antibacterial action.
  2. Anti-inflammatory effect.
  3. Antiallergic effect.

Drops and ointment Sofradeks contain two antibiotics - framycetin and gramicidin, from the group of aminoglycosides. These antibiotics effectively destroy a wide range of pathogenic microorganisms that cause infectious and inflammatory diseases of the ears and eyes. Framycetin destroys a large number of gram-positive bacteria, including Staphylococcus aureus, which is a conditionally pathogenic microbe, often causes inflammatory lesions of various organs, and is rather difficult to cure. Framycetin also has a detrimental effect on the main gram-negative microorganisms - E. coli, dysentery, Proteus, etc. Resistance to this antibiotic is produced very slowly. Framycetin does not destroy streptococci, fungi and microbes that can live in an oxygen-free environment (anaerobic). The second antibiotic, Sofradex, gramicidin, increases the spectrum of action of framycetin, especially acting against staphylococci that are difficult to treat.

The third component of the ointment and drops of Sofradex is the hormone of the glucocorticoid series dexamethasone, which has the property to suppress the inflammatory response, reduce the production of substances that stimulate inflammation, and reduce the degree of capillary permeability. In addition, dexamethasone has the ability to suppress allergic reactions and itching. Dexamethasone also relieves puffiness, improving nasal breathing, hearing and vision.

Sofradex, when injected into the eyes, relieves pain, eliminates burning, tearing and photophobia. And the introduction of the drug in the ears allows you to eliminate redness of the skin, pain, itching, burning sensation in the external auditory canal and the feeling of a stuffed ear.

Indications for use

The indications are the following bacterial diseases:

  1. External catarrhal or purulent otitis.
  2. Bacterial conjunctivitis (inflammation in the surface shell of the eye).
  3. Inflammation of the eyelid and its edges.
  4. Keratitis and iridocyclitis (inflammatory changes in the cornea, iris and ciliary body of the eye).
  5. Sclerites or episclerites (when the skin itself is inflamed or the outer layers of the opaque eye shell).
  6. Eczematid skin infected with eyelid skin (characterized by the appearance of itching, burning and soaking on the skin of the eyelid).

Contraindications

Before starting treatment, you should carefully read the instructions for the drug, as the drops of Sofradex have a number of contraindications and restrictions to use:

  • keratitis caused by the herpes virus;
  • glaucoma (increased intraocular pressure);
  • perforation of the eardrum;
  • pregnancy and breastfeeding;
  • individual intolerance to the components of the drug;
  • diseases of the eye and ear of viral and fungal nature - in this case, the drug will simply not be effective;
  • tuberculosis;
  • trachoma;
  • violation of the integrity of the mucous membrane of the eye;
  • children under 2 years.

With particular care, the drug treatment Sofradeks prescribed to children of younger preschool age, since the long and uncontrolled use of drops can lead to the development of systemic side effects, in particular from the adrenal glands.

Appointment during pregnancy and lactation

To date, experiments aimed at establishing the negative impact of the antibiotic on the course of pregnancy and child development have not been carried out. It is known that the drug can diffuse through the placental barrier and into breast milk. Therefore, pregnant patients and women during lactation are prohibited from using this medication.

With the urgent need to use breastfeeding should be suspended. Important: the active ingredients in the composition negatively affect the intestinal microflora of the child and can lead to the formation of extensive pathological processes.

Dosage and method of use

As indicated in the instructions for use, Sofradex eardrops should be instilled into the external part of the ear canal by 2-3 drops. 3-4 times a day, or - you can lay a gauze pad moistened with a solution.

Drops in the ears of Sofradex apply no more than 7 days, with the exception of cases of visible positive dynamics. For burrowing in the eyes

In case of eye diseases: in case of mild infection, 1-2 drops of the drug are instilled into the conjunctival sac of the eye every 4 hours. In case of a serious infection, the drug is instilled every hour. As the inflammation decreases, the frequency of drug instillations decreases.

Side effect

As a rule, the drug Sofradex is normally tolerated by patients. After instillation of the drug in individuals with increased individual sensitivity may develop local side effects:

  • burning;
  • itching;
  • pain;
  • loss of sensitivity (dull hearing or blurred vision).

These side effects are not dangerous and do not require the abolition of drug treatment and pass on their own after 15 minutes.

Overdose

An overdose of the drug Sofradex is possible in the case of using drops for more than 7 days and its symptoms are due to addiction to dexamethasone. In this case, there is an increase in all the above-described side effects dysfunction of the adrenal glands.

Special instructions

Treatment with drugs containing corticosteroids should not be repeated or long-term without regular monitoring of intraocular pressure, eye examinations for cataracts or secondary infections.

The duration of the drug should not exceed 7 days, except in cases of obvious positive dynamics of the disease, because long-term use of the GCS, which is part of it, can mask hidden infections and the long-term use of antimicrobial components can contribute to the development of sustainable flora.

Framycetin sulfate, which is part of the drug, is an antibiotic from the group of aminoglycosides, which are characterized by the development of nephro and ototoxic effects when applied systemically or topically to an open wound or damaged skin. These effects depend on the dose and increase and renal or hepatic insufficiency. Although the development of these effects was not observed when boiling the drug in the eye, you should take into account the possibility of their occurrence in the case of local application of high doses of the drug in children.

Patients who, after instillation in the eye of the drug temporarily lost clarity of vision, are not recommended immediately after instillation of the drug to drive a car or work with sophisticated equipment, machines or any other complex equipment that requires clarity of vision.

The bottle must be closed after each use. Do not touch the eye with a pipette tip. After opening the bottle, the drug should be used within 1 month.

Interaction with other drugs

Do not use framycetin sulfate with other antibiotics that have ototoxic and nephrotoxic effects (streptomycin, monomitsin, kanamycin, gentamicin).
